Formstack is a no-code online form builder that collects data and carries it straight into the documents, signatures and systems a business already runs on. It is built for organizations where a form starts a process rather than ends one, and where that process still runs on email, spreadsheets and manual rekeying. The platform is made by Intellistack, LLC, the Denver company founded in 2006 as Formstack and renamed in 2025, which continues to sell this line under its original name. The vendor states that more than 32,000 organizations use it. The platform is sold as four connected areas. Formstack Forms builds the forms, with an AI Form Builder that drafts one from a description, Form Prefill that fills known fields from a shared link, and a Theme Editor that applies a company's logos and colors. Formstack Documents populates data into DOCX, PPTX, PDF and CSV templates and delivers the result. Formstack Sign collects digital signatures on any device, with a signing order and links sent by SMS or email. Workflows, the visual builder inside Formstack Suite, ties the three together with step logic and group approvals, while Forms for Salesforce and Documents for Salesforce run the same jobs inside a Salesforce org. Formstack suits mid-sized organizations doing regulated, paperwork-heavy work, and the vendor points at healthcare, education, financial services and non-profits. It fits teams that have outgrown a simple survey tool and need approvals, prefill and a finished record in a system of record. Salesforce administrators are a particular fit, because the Salesforce products are administered inside the org instead of through an external connector. It is bought per Builder user, on plans whose caps on forms, submissions per form and storage rise from one tier to the next. The everyday jobs are patient intake, student enrolment, grant and loan applications, employee onboarding, event registration and any request that needs a signature before it counts. Formstack connects to more than 100 web applications, so a submission reaches a CRM, a spreadsheet or an email tool without a developer, and the Sign Open API covers what the connectors do not. Teams run it beside Salesforce and beside the storage and help desk tools that hold the finished document.
